School Supplies are provided for children in grades Pre-K to 12th grade. Please fill out information below for each child who you would like to register to receive free school supplies. Please give child's full name, school name and grade they will be attending in the Fall of 2020.

Eligibility Requirement Documentation:
To qualify and register for free school supplies, families must complete this registration form and show proof by uploading documentation that their child or children live in the City of Dallas or parents of Dallas ISD students. Families must upload 3 documents to qualify for school supplies.

( Please Note: If you are unable to upload your documents to this registration form, please check the box down below. You will still be required to show proof of the documentation at the event before receiving the school supplies.)

For Dallas ISD students, families must provide proof that the student attends a Dallas ISD school by providing any of the following documentation with student ID number – most recent student report card, letter from student’s school stating the student attends a Dallas ISD school, student ID card OR a printout from the online Parent Portal that shows student’s ID number.

Please upload proof of income or that your family meets the 2020 Poverty Guidelines (Proof of income may be verified using: Paycheck stub from May, June, July or August 2020; a letter dated in May, June, July or August 2020 on agency or company letterhead from an employer, welfare office, caseworker, or Social Security Administration Office indicating annual income and signed by an agent of the company or agency; or a Medicaid, unemployment, disability, or SNAP food benefit letter, card.

Please upload proof of City of Dallas Residency (examples: Utility Bill - showing applicant name and current address from either May, June, July or August 2020; any mail sent to applicant showing applicant name and current address, with a postmark from either May, June, July or August 2020; or letter from landlord or apartment management company, on company letterhead, indicating proof of residency during May, June, July or August 2020.

Once registered, you will receive an email with your confirmation number. Please note: You will need to show your confirmation number to the MBTSF volunteers at the event in order to receive your school supplies. Prior to August 20th, you will receive another email with arrival details and other updates for the MBTSF.
